Skilled Nursing Facility. We specialize in short term rehab and long-term care. We provide respite care as well.

Yes! Please call 817-444-2536 and speak with Bridgette Maclin for more information on Medicaid Services.

What other insurances do you take?

Medicare, Medicaid, Medicaid Pending, Private Pay, Care-N-Care Medicare Replacement, Amerigroup Medicaid, Cigna Medicaid, Molina Medicaid. United Healthcare and Blue Cross Blue Shield Medicare are coming soon!

How can we volunteer at Azle Manor?

What is the cost per day for your facility?

Semi private room is $170.00 per day Private Room is $325.00 per day

What is included in the daily cost?

We provide 3 meals per day, 24 hour nursing care, laundry services, daily activities.

What are you visiting hours?

9am to 5pm Monday- Sunday

What are you covid protocol?

We adhere to HHSC and CDC protocol guidelines that are forever changing.

How do we make a payment and when?

We accept cash, check or cc with a 4% fee. Payment are due by the 15th of each month
